Start the Admission Process

The Poinciana Rehabilitation Centre (PRC) is committed to making the admission process as clear and supportive as possible for individuals, families, and referring professionals.

Our clinical and administrative team is experienced in guiding applicants through the process and collecting the necessary information required to assess eligibility for admission. We work closely with families, healthcare providers, and community partners to ensure that each referral is reviewed carefully and that applicants receive appropriate support throughout the process.

Who Can Apply

Applications and referrals may be submitted by:

• Individuals seeking rehabilitation services
• Family members or caregivers
• Psychiatrists or medical practitioners
• Hospitals or healthcare providers
• Government agencies and social service organizations
• Court-designated referrals or other authorized agencies

All referrals are reviewed by the PRC clinical team to determine suitability for admission.

How to Apply

Step 1 – Download the Referral Form

Complete the PRC Referral Form, ensuring that all required sections are filled out and relevant clinical information is included.

Step 2 – Submit the Completed Form

Completed referral forms may be submitted by email or delivered directly to PRC.

For confidentiality and data protection purposes, all electronic submissions are encrypted.

What Happens After Submission

Once the referral form is received:

1. The PRC clinical team reviews the referral documentation.
2. Additional information may be requested from the referring professional if necessary.
3. The applicant may be invited for further clinical assessment or screening.
4. If the applicant meets the admission criteria, an admission date and treatment plan will be arranged.

The review process ensures that PRC can provide a safe and appropriate therapeutic environment for all residents.
