Overview

At Poinciana Rehabilitation Centre (PRC), we are committed to continuously improving the quality and effectiveness of the care we provide. Monitoring outcomes helps us understand how residents are progressing, evaluate the impact of our programs, and ensure that our services support meaningful recovery.

PRC uses structured assessments and regular progress monitoring to evaluate residents’ clinical, social, and functional improvement during their stay.

When residents are admitted, baseline information is collected to help guide treatment planning and measure progress over time. This may include:

  • Diagnostic assessments
  • Clinical and psychosocial history
  • Individual treatment goals
  • Medication and health information
  • Functional and behavioural observations

Throughout a resident’s stay, multidisciplinary teams monitor progress through regular reviews, clinical observations, and participation in therapeutic programs.

These may include:

  • Participation in therapeutic activities and rehabilitation programs
  • Engagement in social and community activities
  • Clinical and behavioural progress reviews
  • Resident feedback and satisfaction surveys
  • Quality-of-life and recovery-focused assessments

Progress is reviewed periodically to ensure that treatment plans remain appropriate and responsive to the resident’s needs. This approach helps clinicians adjust interventions, strengthen recovery planning, and prepare residents for successful reintegration into the community.

Outcome monitoring is also used to strengthen PRC’s service delivery by informing programme development, quality improvement initiatives, and clinical best practices.
