Monitoring Recovery & Discharge Outcomes

An important measure of success at PRC is how well residents are prepared for the next stage of their recovery journey. Throughout a resident’s stay, the multidisciplinary team works closely with the individual to support progress toward recovery goals and readiness for transition to a less restrictive environment.

Discharge planning begins early and forms part of the resident’s overall treatment and rehabilitation plan. The process focuses on ensuring that residents have the support, stability, and resources needed to continue their recovery safely in the community.

Key areas considered when evaluating readiness for discharge may include:

  • Clinical stability and symptom management
  • Ability to manage daily living activities and personal care
  • Adherence to treatment and medication plans
  • Development of coping strategies and relapse prevention plans
  • Engagement with community supports and services
  • Family involvement and support networks where appropriate

PRC works collaboratively with community partners, healthcare providers, and support services to help ensure continuity of care after discharge.

Monitoring recovery outcomes helps the Centre assess how effectively residents transition into community living and informs ongoing improvements to rehabilitation programmes and support services.
